PHQ-9 is a screening and symptom-severity tool. A score does not by itself diagnose depression, and item 9 requires attention regardless of the total score.

Each of nine symptoms is rated from 0 to 3 according to how often it occurred during the past two weeks. The total ranges from 0 to 27. The interactive questionnaire is currently available on the Japanese page.
A screening result is not a diagnosis
A clinician also considers symptom duration, functional impact, physical illness, medication, alcohol use and any history of manic symptoms. Seek care whenever symptoms are distressing or interfere with daily life, even if the total score is low.
